We provide holistic, comprehensive case management and service coordination in a linguistically and culturally sensitive way. Our service coordination includes benefits and services provided by state and federal agencies as well as other social service agencies.

The Process
- Intake & Assessment – We’ll listen to your needs and gather basic information.
- Case Support – A staff member will provide counseling and connect you with in-house or external services.
- Follow-Up – We’ll check in with you to ensure your needs are being met and help resolve any ongoing challenges.
