GUIDE Model
Comprehensive Dementia care, close to home.
Details
CASL is proud to be a provider of the Guiding an Improved Dementia Experience (GUIDE) Model, a nationwide initiative from the Centers for Medicare & Medicaid Services (CMS). The GUIDE Model brings together medical care, caregiver support, and community resources to improve the lives of people with dementia and those who care for them.
Who Can Participate?
- Medicare Part B beneficiaries with a dementia diagnosis.
- If no formal diagnosis exists, CASL’s dementia-trained Nurse Practitioner can provide evaluation and attestation.
- Unpaid caregivers of participants are included, regardless of their own insurance or income.
How It Works
- Contact Us – Email Alzheimers@CASL.org, call (312) 791-0418, or submit the form below.
- Check Eligibility – Share your insurance and ID; CASL confirms Medicare Part B coverage.
- Assessment & Care Plan – Care Navigator, Nurse Practitioner, and occupational therapist create a personalized plan.
- Start Services – CASL coordinates dementia care, caregiver education, and community supports.
- Ongoing Support – Regular check-ins and annual reassessments keep care up to date.
